Budget Cuts – Who is to Blame?

I recently submitted and received approval from our Board for next year’s budget. As part of the communication, I sent the following email to all employees:

Our Board of Trustees met today and approved the Fiscal Year (FY) 2011 budget. About a year ago, I sent a memo to all staff offering context and important details about the FY 2010 budget and I have decided to continue that practice again this year.

Current Environment

I want to begin by reviewing the context in which the FY2011 budget has been developed. At Town Hall Meetings and in other venues, I have talked and written about the impact of current economic conditions on GHS and our operating margin. Here are some of the facts we have discussed:

The United States has experienced the deepest recession since the Great Depression of the 1930s and most signs of recovery this year have been sporadic and sluggish.

Financial and housing markets have seen dramatic declines and have only achieved partial recoveries.
Unemployment across the country continues at levels not seen for nearly three decades.
In our own state, unemployment is nearly 11 percent; and that places South Carolina’s among the very highest unemployment rates in the country. The majority of people in South Carolina receive health care coverage from their employer.

At GHS, we have continued to experience lower than projected inpatient volumes and shifts in payor mix including rising charity care and bad debt. These changes represent a new normal for GHS and we now project flat inpatient volumes and modest growth in outpatient volumes over the next few years.

In FY 2010, GHS management and staff have responded to these challenges by seeking opportunities to improve efficiency and reduce costs and I know that we will continue to face and respond to similar challenges in the coming year.

GHS Strategy and 2011 Goals

Our new fiscal year starts on October 1st and these factors impact strategy, goals, and the budget for the coming year. You may remember that in FY 2009 our Board of Trustees approved changes to our vision and mission statements that reflect the strategic direction that GHS has been following for several years.

In 2011, we will move forward with selected strategic initiatives that are key to our long-term strength as an important health resource for our community. These include expanding access to primary care through our physician network, investing in clinical expertise, programs and information technology, and supporting progress on the University of South Carolina School of Medicine – Greenville expansion.

In 2011, our organizational goals relate to the six GHS Pillars of Excellence. These are:

People – We work to transform health care, measured by our annual staff survey.
Service – Patients and families are the focus of everything we do, measured by our patient surveys.
Quality –.We provide right care at the right time and in the right place, measured though CMS All Care Measures, Hand Hygiene and Culture of Safety data.
Growth – We develop our System to meet the needs of our community, measured by net revenue and numbers of new patient visits.
Finance –We responsibly direct our resources to support our mission, measured by our operating margin.
Academics –We educate to transform health care, measured in 2011 by achieving preliminary accreditation of the USCSOM – Greenville.

In developing any budget, leadership must balance the future and strategic priorities of our health system with the current needs of the organization. This year was no exception. In addition to the strategic initiatives described above, we identified two additional budget priorities: 1) minimizing costs to the community while budgeting an operating margin that maintains financial stability; and 2) protecting and growing jobs.

More than ever before, this year’s budget process benefited from the additional input and involvement of our physician leaders/clinical chairs and campus presidents who considered strategic system priorities and helped make tough decisions about a wide range of issues including capital investments, staffing levels, and improved integration of services.

For many of the reasons I’ve already described, the FY 2011 budget reflects some difficult decisions which will touch each of us as employees.

The budget I submitted provides for no across-the-board merit increases in 2011. This applies to all eligible employees, physicians, senior leadership and myself. Funds will be made available for market adjustments and to support employee promotions to new positions. It may be worth noting that GHS continues to be market competitive in its employee compensation practices.

We will maintain a comprehensive and competitive benefits program; however, the cost of benefits will rise for employees at a pace that is lower than the national average. For example, insurance premiums will increase an average of 4% and co-pays and deductibles will increase slightly.

In making these decisions a theme emerged around shared sacrifice to protect jobs. I want you to know that the combined effect of these decisions equals nearly 260 positions protected. I understand that these budget decisions will have a personal impact for you and your family. I encourage you to consider the many colleagues whose jobs have been protected as you think about the implications of this budget on your individual situation.

Town Hall Meetings

Last week I finished another round of Town Hall Meetings. I thought these meetings may be of interest from an employee engagement and a content perspectives.

My belief is that an engaged workforce comes from ensuring that the staff knows what leadership and the organization are doing and why. It is important for everyone to understand the context for the decisions and to hear the rationale behind various actions – even if there is disagreement it is good for employees to have a voice. As a result, we started a systematic way for me to communicate and interact with employees. The Town Hall Meetings are one example of that practice. Here is a brief overview. I conduct 10 – 12 sessions each quarter and total attendance ranges from 1,500 – 1,800 employees with sessions held on each campus, including the night shift.

The agenda includes updates on strategy, system-wide goals (Pillars: People, Service, Quality, Growth, Finance, and Academics – the same information reported to the Board) and other timely topics. In addition, we close-the-loop on open issues or questions from previous meetings and request feedback via the Town Hall Surveys which include a subset of key questions from the annual Employee Opinion Survey – this provides a quarterly “pulse” of the organization. Preparation for the meeting has been very valuable for me. My experience is that whenever I have to teach or explain something, I learn it more deeply. That learning has helped me better understand the organization.

I invite you to take a look at the information we discussed at the last set of Town Hall Meetings. The format is fairly standardized and this was the 16th time I have held these sessions. Here is a link to the August Town Hall Meeting presentation. In particular, take a look at the presentation by Howell Clyborne dealing with the South Carolina economy. That is an example of giving employees a context for what is happening at GHS. I also believe that for the next several years the economy is the bigger story than Health Care Reform.

Organizational Goals and Pillars

In the prior entries I reviewed our vision and mission statements, today I want to write about Goals and Pillars As a reminder, our Vision is to Transform Healthcare for the benefit of the people and communities we serve and our Mission is to Heal Compasionately, Teach Innovatively and Improve Constantly. Vision and mission are critical, but the goals are what bind everything together.

Have you ever had fun, or not, making New Year’s resolutions? My experience is that the creation of goals for a new year are most productive and doable when I have a longer view of where I am headed and they are connected to my personal vision and mission. I believe the same principle applies to creating goals for an organization, there is a need for a clear vision and mission.

Here are some questions I ask:

1. What is seeking to emerge? What is supposed to come forth, improve, manifest and to what end?
2. What must I/we become to achieve it?
3. What strengths or assets do I currently possess to accomplish the vision.
4. What has to change or improve or in order for that to happen?

It is with these questions in mind that the annual goals are developed. Again, they can be personal goals or organizational goals, the process is the same.

At GHS we start with the big idea, what we believe will transform how we deliver care and we call that Total Health. Total Health provides the overarching framework for our goals, it is what is seeking to emerge from us, and it sits atop our goals document. The short definition of Total Health is delivering the right care, in the right place at the right time.

As you looked at the Total Health slide, you could see our Pillars, Pillar Statements, and the Measures. We have six Pillars: People, Service, Quality, Growth, Finance and Academics. These are our areas of focus. I believe we have created a culture of measurement and improvement and the goals drive that improvement. I also think the more open we are about discussing our goals increases the likelihood of success.

As you look over the goals under the pillars a couple of things may emerge. First, when there is national benchmark available we compare ourselves against how others are doing. Second, we want to be a best performer so we strive to be in the top quartile – meaning we want to do better than 75% of all the hospitals we compare ourselves to. For some measures that is a stretch and for others, like Employee Commitment and Quality, we are in the top 10%. Another thing to note is that we are moving more towards multi-year goals. I will come back to multi-year goals in a future entry and why I think they are important. I will also share my personal goals.

I welcome any thoughts, comments or blurts about these goals. I plan to discuss them in more detail in the future, as well as my personal goals.

9th Circuit Drives Small Business Owners out of San Francisco

The title of this post is a prediction rather than a description.  The New York Times reports that a three judge panel from the U.S. Ninth Circuit Court of Appeals granted the city of San Francisco, CA a temporary reprieve from a lower court ruling that would’ve prevented the city from forcing small business owners to pay $1.17 to $1.76 per hour, per employee for health care expenses…regardless of whether they already provide employees health care.  While the case is formally appealed to the Ninth Circuit, anyone who owns a business with more than 20 employees must pay the fees, which can go toward a variety of health-care options, including employer-provided insurance, health savings accounts, direct payment of medical bills, or payment in a new city program called Healthy San Francisco. Some employers, however, say the plan places an undue burden on smaller businesses, many of which are already paying for employee benefits. “It’s expensive, it’s unsustainable and there’s better ways to do it,” said Daniel Scherotter, the incoming president of the Golden Gate Restaurant Association, which filed the suit challenging the law.

Mr. Scherotter, who owns an Italian restaurant in the city, estimates that he already spends $60,000 a year on health insurance, but that the new plan could cost him twice that.

“Everybody seems to know that restaurants are really risky business, but somehow, they’re saying, ‘Oh, they’re rolling in it, they can pay for it,’ ” he said.

Will San Francisco solve the nation’s uninsured health care crisis or will it drive away small businesses because paying the expenses will make them unprofitable?  Well, the only way to find out is if the appeal is lost.

Unfortunately, the lawsuit is based upon a “conflict of laws” theory (whether the City’s ordinance conflicts with or attempts to trump a Federal law), so if the Supreme Court agrees to hear it, it won’t be directly ruling on the notion of employer-sponsored health care.  However if it does get to the Supreme Court of the United States, we may get some interesting dicta about the notion of employer-based (funded) health care and whether this sort of welfare program has a chance with other city or state governments.

The funny thing is - the added expense will ultimately be passed down to the employees of these business and the consumers, the very people who will be utilizing the free health services in the first place.  These people could take it upon themselves to buy health insurance…a much cheaper alternative than a City-built infrastructure.

What Makes You The Target Of Mosquitoes?

Mosquitoes are one of the biggest threats to spring and summer fun. A bite on the skin leaves behind an itchy red welt and possibly even something more serious like an illness. As you swat crazily at these pests, you notice that other people around you are not being targeted. Why?

Mosquitoes prefer to bite certain people over others.  1 in 10 people are the most targeted candidates for mosquito bites, according to Jerry Butler, Professor Emeritus at the University of Florida. But it is not dinner that they are trying to suck out of you. Female mosquitoes need human blood to develop fertile eggs and not everyone’s blood will help them achieve their goal.

Scientists have been studying the triggers of mosquito bites for years, especially the compounds and odors we exude. Although they are still going through the 400+ compounds and odors and testing them, scientists are certain that genetics account for 85% of mosquito bites.

People with high concentrations of steroids or cholesterol on their skin surface attract more mosquitos, experts argue. As well, people with excess amounts of acids produced, such as uric acids, are also more susceptible. These substances trigger a sense of smell in the pests, luring them to land on the unsuspecting victims.  Another recommendation for avoiding the onslaught of mosquito bites is to try to keep to one spot as opposed  to moving around because mosquitos sense movement and will heard towards you.

How to Keep the Bites at Bay?

There are plenty of mosquito repellents being sold in drugstores, convenience stores and supermarkets. It’s important to understand that they are not all created equally.  The best products are ones with DEET. DEET continues to be the chemical of choice used in repellents successfully for over 45 years.  If you have children aged 2 years of age or  less, it is recommended to have a low concentration of DEET i.e. 10% or less.

Other Ways to Repel Mosquitoes:

Wear light-colored clothing; dark-colored clothes attract these pests
Wear long sleeve shirts, pants, socks and hat; especially when sitting on the patio or in the shade.
Discard standing waters around the home as these pests are known to breed in water.
Ensure all doors are closed quickly during the spring and summer months.

When is the Best Time to Exercise?

When is the best time to exercise? That is a question we ask ourselves very often. Here’s a breakdown of every time frame, and its advantages and disadvantages, to help you make the right decision. It is important to note that the workout itself is what really counts. Doing a workout when it is most convenient for you is preferred over no workout at all.

Morning Workout

Advantages:

  • The meals you eat after your workout will be absorbed for muscle recovery and for the creation of new muscle tissue. This reduces the chance for fat storage as the nutrients inside the food you eat are being used up.
  • After your workout the body releases a hormone called serotonin, which is responsible for contentment. If you exercise early in the morning, you will start your day in a great mood.
  • Your morning workouts tend to be more productive as there are fewer people in the gym then. You will not have to wait to use the machines or free weights, which helps if you want consistency.
  • You are more likely to stick to your workouts when done in the morning, as there is less of a chance that “unexpected events” will interrupt your training or cause you to abruptly leave.

Disadvantages:

  • If you are not a morning person you might not have the energy to exercise. This will significantly impact the intensity of your workout and chances are you will not be able to complete everything you intend to do.
  • Your blood pressure is highest in the morning. So, if you have any heart problems, you might want to move your workout later in the day.
  • After an intense workout your muscles need to rest. If you are immediately going to work after gym and the job requires physical work, morning exercise may not be for you.

Afternoon Workout

Advantages:

  • Body temperatures are at their highest, meaning that there is less chance of an injury. It is still recommended to complete warm-ups before the start of any exercise program.
  • Mental focus and physical functioning are at an optimal level, you are not too tired to get out of bed and not too exhausted from a hard day at work. This gives you the strength and endurance you need to maximize workout.
  • Pain sensitivity is at its lowest level, which means that you can push yourself harder and train more intensely.
  • An afternoon workout helps you de-stress and take a break from your daily, and sometimes stressful routine. You will feel refreshed and more energetic to go back to the unnerving work and complete unfinished tasks.

Disadvantages:

  • Similarly to the morning workout, if your training is followed by heavy physical activity, you will need to give your body some time to recover.
  • An afternoon workout may not be ideal as people are not able to get time off work or studies.

Evening Workouts

Advantages:

  • Good for muscle size as the workout is often followed by rest or sleep.
  • Performance is considerably high in the evening, even though it might feel you need to provide more input.
  • Depending on your body and how well it adapts, you might find it easier to fall asleep after an evening workout. However, you may also experience the opposite effect – insomnia.

Disadvantages:
  • You may not be able to provide your body with 2-3 meals after the workout, which may somewhat impact muscle building.
  • Unexpected events/emergencies or low energy levels can act as obstacles to you finishing all your tasks in your exercise program.
  • Gyms are usually packed in the evening, which means that you will often have to queue for machines, weights or other equipment.
  • Evening workouts can interfere with your sleeping patterns as it increases your adrenaline levels, heart rate and body temperatures.

In the end, it is most important to find a realistic, consistent workout schedule, no matter what the time. It is better to have a workout at your most convenient time, then to have no workout at all.

One Response to “Hospitals Will Have to Pay for Mistakes”

s a registered nurse working in a large urban teaching hospital I’d like to comment on the 1/15/08 article that appeared in the Wall Street Journal regarding insurance companies no longer assuming the costs for preventable errors in hospitalized patients. With today’s emphasis on educating the consumer and soaring healthcare costs it was inevitable that sooner or later someone, the Federal government or the insurance companies, would devise a plan to control their costs. It is a fact of life today as a healthcare provider/organization that you either “adapt or die”. Change is always difficult and constant change is overwhelming and often has the effect of desensitizing the healthcare professional. I choose to reframe the issue and see it as advocating for best practice outcomes in patient care and not as an overall attack against the healthcare industry.

There are some areas in healthcare that we should be more proactive in preventing! In order to do that we need to constantly think outside the box and be vigilant in keeping one step ahead by staying current in the latest trends and research, becoming involved in advocating for patient safety. We need to promote and encourage organizations to trial new products as they arrive on the market such as the Vivax Mobility System and the Soma Safe Enclosure Bed. Both are manufactured by the Vivax Medical Corporation. The Soma Safe Enclosure Bed helps prevent patient injuries from falls and wandering, two of the items on the list that are non-reimbursable. It took my organization eighteen months to finally implement the Soma Safe Enclosure Bed and another six months to get the staff to utilize it. Once it took hold it has been a win-win situation for staff and patients alike. The same hold true for the Vivax Mobility System as a means to get patients up and help to decrease the incidence of decubitus ulcers, catheter infections and pneumonia.

Change is difficult and we must learn to embrace it if we are to stay viable! My last point is in regards to what Michael Maves, executive vice president of the American Medical Association, said in his letter to CMS. I too share a concern that we as an industry will be penalized for “complications that are a biological inevitability”. My belief and hope is that the Federal government and the insurance companies will continue to offer the healthcare industry “due process” in the form of case review and the denial appeals process as a means tease out what is truly preventable and what is inevitable. If this cannot be sufficiently accomplished then I fear that the patients in greatest need will be discriminated against and patient centered care and improved patient outcomes will become a pipedream.

The consumption of sugary drinks has increased over the past 3 decades in the United States, with approximately 50% of the population drinking the beverage daily.  Such drinks have been linked to a greater risk of obesity, diabetes, cardiovascular diseases and cancer.  A 2011 study lead by Professor Gitanjali Singh, research assistant professor at Friedman School of Nutrition Science & Policy of Tufts University in Boston, MA, now also claims that sugary drinks are also associated with over 184,000 adult deaths in the U.S. each year.

For the study, Professor Gitanjali Singh and his colleagues set out to determine the annual rates of global deaths and disabilities caused by sugary drink consumption. They analyzed 1980-2010 data from 62 surveys completed by 611,971 people in over 51 countries. The focus of the study was on how the consumption affected the number of deaths from diabetes, cardiovascular diseases and cancer.  It also excluded pure fruit drinks and all drinks with at least 50 kcal per 8 oz. servings (U.S.).

Key Findings:

  • Young adults who consume sugary drinks were more likely to experience chronic diseases than older adults
  • Low- & middle-income countries had the highest estimated rates of deaths due to sugary drinks consumption; approximately 76% of the deaths occur in these regions.
  • Mexico had the highest rate of estimated deaths related sugary drinks with 405 deaths per 1 million adults among the 20 most populated countries.
  • The United States had the second highest rate, with an estimated 125 deaths per 1 million adults.
  • 133,000 deaths from diabetes, 45,000 deaths from cardiovascular diseases and 6,460 deaths from cancer were caused by fizzy drinks, fruit drinks, energy drinks and sweetened iced teas in 2010 (worldwide stats).

All their findings can be found in more detail in Circulation, a journal that is published by the American Heart Association (AHA).

The American Heart Association (AHA) recommends consuming no more than 450 calories from sugar-sweetened beverages each week – that is less than three 12 oz cans of cola.

Are Free Markets a False Idol? Or is Regulation a Trojan Horse?

Peter Goodman of the New York Times writes an interesting retrospective of free markets entitled “The Free Market: A False Idol After All?“  The article’s basic conclusion is that unfettered free markets (unhampered by regulation)…systems merely based upon ideology, cannot stand without some basic rules of government.  And I don’t disagree.  In the short run, the market can be as fickle and irrational as Lindsay Lohan after a few Vicadin, some blow, and a rude text from her boy-toy of the week.  Just watch any blue-chip stock get slaughtered after missing some analyst’s quarterly earnings estimates by 2 cents.  But Adam Smith’s invisible hand, in the long run, will put resources where they can be invested most efficiently…creating the most value.  And I truly believe that.

The problem illuminated by Goodman’s article isn’t that we have government regulation…it’s that the same knee-jerkedness that renders a short term market stupid is what’s driving legislation through Congress.  Politicians are elected for a term of years, not decades, and in order to make a difference…an elected official must be able to pass a bill quickly, no matter how hastily researched or ill-conceived.  Republicans and Democrats alike are killing America’s competitiveness in an increasingly globalized world.

The latest disgrace is the Bush Administration’s efforts to freeze adjustable mortgage rates for families who cannot afford the contractual step-ups in rate that will soon torpedo many of them into foreclosure.   Bill Gross, who manages one of the largest bond funds in the world at PIMCO (Pacific Investment Management Company), is one of the most brilliant minds in the world when it comes to markets and capturing value…and even he is pro-bailing out the American Idiot.  Now, Gross, who is quite possibly the greatest bond trader alive today, may have some super-complicated, multiple-degrees-of-separation reason for why this will create value that none of us can see…but based upon the interviews he has given, and the articles he has written, it’s just plain ole bleeding heart self-defeating behaviour.

To bail out people who borrowed more than they can afford is to teach America that the government will always be there to bail them out.  There’s no way a society who doesn’t have to develop its own good judgement will ever compete in a global economy.

To quote the article: “Every regulation reduces people’s freedom,” said David R. Henderson, a libertarian economist at Stanford University’s Hoover Institution. “The more regulation we get, the worse we do.”

Enron and Worldcom cooked their books and stole from their employees.  The government’s knee-jerk reaction was Sarbanes-Oxley, which is so cumbersome and expense to comply with, that companies are leaving the U.S. rather than spend the billions required to comply.  Two bad apples (really bad apples, granted), sparked hastily drafted regulation that is making good companies so uncompetitive in a global market, that they are actually leaving the country for freer markets.

There’s a fantastic, tongue-in-cheek article written by former bond trader and author of “Liar’s Poker,” Michael Lewis, where he jokes about how an overly paternalistic America has dared him and his wealthy, productive friends to up and move to Dubai.  His point is that pandering politicians can scapegoat successful people, because they are easy targets, and work to tax the hell out of them…b/c they must be doing something evil…making so much money in the first place.  How can these select few entrepreneurs and hedge fund managers earn so much money?  They must be unethical and worthy of our scorn.  As soon as someone makes $100mm, a mentality has been created that has trained people to try to pull them down.  Its driven by jealousy, guilt, and in the case of people like Hillary Clinton, pathological ego (and hypocrisy).  Michael Lewis ends his comedic piece daring the IRS to tax him more…he’ll always stay a step ahead.  And if not, its a mobile world.  There are plenty of places with low taxes who will encourage the forward-thinkers, the innovators, thecreaters of wealth and value.  Are free markets a false idol, or is the belief in the notion an expression of envy and a lack of foresight?

If these sentiments trickle into health care, we will be a third-world, possibly Arabic-speaking country withing a few generations..

Coronary Artery Disease In Women

Coronary artery disease (CAD) is a medical condition caused when plaques build up in the walls of the coronary arteries. These plaques can gradually obstruct the artery, disturbing the continuous supply of oxygen and nutrients to survive. CAD is caused by atherosclerosis, which is a chronic, progressive disorder of the arteries in which deposits of cholesterol, calcium, and abnormal cells build up on the inner lining of the arteries.

More women die of cardiovascular disease than of any other cause, and most of these deaths are due to coronary artery disease (CAD). CAD can be particularly damaging to women because the symptoms of CAD can be different in women and the standard methods of diagnosing CAD can be misleading in women. Instead of chest pain, they are more likely to experience a hot or burning sensation, or even tenderness to touch, which may be located in the back, shoulders, arms or jaw. Often women have no chest discomfort at all.

Again, the diagnostic tests that work quite well in men can be misleading in women. The ECG test in women during exercise can often show changes suggesting CAD, whether CAD is present or not, making the study difficult to interpret. Many cardiologists routinely add an echocardiogram or a thallium study when doing a stress test in a woman, which greatly improves diagnostic accuracy.

In women with typical CAD, coronary angiography can identify the exact location of any plaques (i.e., blockages) within the coronary arteries, but in women with atypical coronary artery disorders, coronary angiograms often appear misleadingly normal. As such, if the physician trusts the results of the angiogram, he is likely to miss the real diagnosis.
